INJECTABLE FLU SHOT SCHEDULE (2010)*
All dates are evening clinics held 6 – 7:30pm, except for daytime clinics noted below
TO SAVE TIME, PLEASE DOWNLOAD THE WILLOWS INJECTABLE FLU SHOT ADMINISTRATION SHEET, FILL IT OUT, AND BRING IT WITH YOU WHEN YOU COME FOR YOUR FLU SHOT
(Please remember that if someone other than a parent/guardian is bringing a child in for the flu vaccine, a written note giving permission to give the vaccine signed by a parent will be required.)
TIER I:
High risk children and their siblings, all patients exposed to infants <6 months, and children 6 months through 5 years:
- 9/22 Wednesday 10am – 12 noon
- 9/23 Thursday 12noon – 2pm
- 9/28 Tuesday
Or any of the dates following
TIER II:
Children and Adolescents 6 years and older:
- 9/30 Thursday
- 10/6 Wednesday
- 10/7 Thursday
- 10/11 Monday Columbus Day: 10am – 12 noon and 1pm – 3pm
- 10/12 Tuesday
Or any of the dates following
Additional Flu shot clinics for any patient and follow-up vaccines as necessary:
- 10/20 Wednesday
- 10/28 Thursday
- 11/2 Tuesday Election Day: 10am – 12noon and 1pm – 3pm
- 11/9 Tuesday
- 11/18 Thursday
- 11/23 Tuesday
Please note that you may “walk-in” during the above times and you will be seen on a first come first serve basis, no appointment necessary. You must be a patient of Willows Pediatric Group and show a valid insurance card or be prepared to pay at time of service.
